ABSTRACT

The present study was conducted to isolate and characterize bacteria from water and soil sample taken from the Lahore Canal at different sites i.e. Mall Road, Mohlanwal and Khera site. Isolated bacterial strains were identified on the basis of morphological and biochemical tests. Identification was confirmed by culturing bacteria on selective media. Antibiotic resistance test was also performed to observe the resistance of bacteria against different antibiotics. Blood agar test was performed for identification of different pathogenic bacteria. The result revealed that water and soil samples of Lahore Canal Lahore from different sites were contaminated with Escherichia coli, Salmonella sp., Vibrio sp., Bacillus spp., Enterococcus sp. and Staphylococcus spp. Due to presence of these pathogens, this water is not suitable for any domestic and irrigation use. Study also revealed that water of the Lahore Canal is harmful for human health as it is contaminated with bacteria that can cause severe disease e.g., Escherichia coli can cause gastroenteritis, Bacillus spp. can cause nausea and vomiting, Enterococcus may infect urinary tract, Salmonella sp. is responsible for Bacteremia, Staphylococcus spp. can cause mild fever and Vibrio sp. can be the reason of cholera. Thus it is rendered unfit for any kind of human use even other than drinking like swimming, bathing, washing etc., until and unless some remedial measures are employed to eradicate pathogenic microorganisms by WASA and LWMS according to standards of WHO. Similarly, it is quite harmful, when and where ever it is used for irrigation without proper treatment.


O presente estudo foi realizado para isolar e caracterizar bactérias de amostras de água e solo retiradas do Canal Lahore, em Lahore, em diferentes locais, ou seja, Mall Road, Mohlanwal e Khera. As cepas bacterianas isoladas foram identificadas com base em testes morfológicos e bioquímicos. A identificação foi confirmada por cultura de bactérias em testes de meios seletivos. O teste de resistência aos antibióticos também foi realizado para observar a resistência das bactérias a diferentes antibióticos. Foi realizado o teste de ágar sangue para identificar diferentes bactérias patogênicas. O resultado revelou que amostras de água e solo do Canal Lahore, Lahore, de diferentes localidades estavam contaminadas com Escherichia coli, Salmonella sp., Vibrio sp., Bacillus spp., Enterococcus sp. e Staphylococcus spp. Por causa da presença desses patógenos, essa água não é adequada para qualquer uso doméstico e de irrigação. O estudo revelou que a água do Canal Lahore é prejudicial à saúde humana, pois está contaminada com bactérias que podem causar doenças graves, por exemplo: Escherichia coli pode ocasionar gastroenterite; Bacillus spp. pode causar náuseas e vômitos; Enterococcus sp. pode infectar o trato urinário; Salmonella sp. é responsável pela bacteremia; Staphylococcus spp. pode causar febre leve; e Vibrio sp. pode ser a razão da cólera. Assim, torna-se imprópria para uso humano, como natação, banho, lavagem etc., até que algumas medidas corretivas sejam empregadas para erradicar microrganismos patogênicos por WASA e LWMS de acordo com os padrões da OMS. Da mesma forma, é bastante prejudicial, quando usada para irrigação sem tratamento adequado.

ABSTRACT

Introduction. Inadequate prescription of antibiotics has been recognized as a public health problem by the World Health Organization. In this context, antibiotic stewardship programs have been implemented as a tool to mitigate its impact. Objective. To describe the changes in clinical outcomes after the implementation of an antibiotic stewardship program in a level IV hospital. Materials and methods. We conducted a unique cohort study of patients hospitalized for infectious pathologies that were treated with antibiotics in an advanced medical facility. We collected the clinical history before the implementation of the antibiotic stewardship program (2013 to 2015) and then we compared it to the records from 2018 to 2019 collected after the implementation of the program. We evaluated changes in clinical outcomes such as overall mortality, and hospital stay, among others. Results. We analyzed 1,066 patients: 266 from the preimplementation group and 800 from the post-implementation group. The average age was 59.2 years and 62% of the population was male. Statistically significant differences were found in overall mortality (29% vs 15%; p<0.001), mortality due to infectious causes (25% vs 9%; p<0.001), and average hospital stay (45 days vs 21 days; p<0.001); we also observed a tendency to decrease hospital re- admission at 30 days for infectious causes (14% vs 10%; p=0.085). Conclusions. The antibiotic stewardship program implemented was associated with a decrease in overall mortality and mortality due to infectious causes, as well as in average hospital stay. Our results evidenced the importance of interventions aimed at mitigating the impact of inadequate prescription of antibiotics.


Introducción. La inadecuada prescripción de antibióticos es un problema de salud pública, reconocido por la Organización Mundial de la Salud. Los programas de gestión de antibióticos son implementados como una herramienta para mitigar su impacto. Objetivo. Describir los cambios observados en los desenlaces clínicos después de la implementación de un programa de gestión de antibióticos en un hospital de IV nivel de atención. Materiales y métodos. Se llevó a cabo un estudio de cohorte única de pacientes hospitalizados por patologías infecciosas y tratados con antibióticos en una institución médica de alta complejidad. Inicialmente, se recolectaron las historias clínicas anteriores a la implementación del programa de gestión de antibióticos (2013 a 2015) y luego se compararon con los datos obtenidos después de la implementación del programa de gestión de antibióticos de 2018 a 2019. Se evaluaron los cambios en los desenlaces clínicos como mortalidad y estancia hospitalaria, entre otros. Resultados. Se analizaron las historias clínicas de 1.066 pacientes: 266 con historia previa a la implementación del programa y 800 con historia posterior a la implementación. El promedio de edad fue 59,2 años y 62 % de la población era masculina. Se encontraron diferencias estadísticamente significativas en mortalidad global (29 Vs. 15 %; p<0,001), mortalidad por causa infecciosa (25 % Vs. 9 %; p<0,001) y promedio de estancia hospitalaria (45 Vs.21 días; p<0,001), con tendencia a disminuir nuevas hospitalizaciones en 30 días por patología infecciosa (14 Vs.10 %; p=0,085). Conclusiones. El desarrollo del programa de gestión de antibióticos se asoció con a una disminución en la mortalidad global, la mortalidad por causa infecciosa y la estancia hospitalaria. Esto demuestra la importancia de desarrollar intervenciones dirigidas a mitigar el impacto de la prescripción inadecuada de antibióticos.

Introducción. En pacientes con diagnóstico de colecistitis aguda tratados con colecistostomía, el tiempo óptimo de duración de la terapia antibiótica es desconocido. El objetivo de este trabajo fue comparar los resultados clínicos en pacientes con diagnóstico de colecistitis aguda manejados inicialmente con colecistostomía y que recibieron cursos cortos de antibióticos (7 días o menos) versus cursos largos (más de 7 días). Métodos. Se llevó a cabo un estudio de cohorte observacional, retrospectivo, que incluyó pacientes con diagnóstico de colecistitis aguda manejados con colecistostomía, que recibieron tratamiento antibiótico. Se hizo un análisis univariado y de regresión logística para evaluar la asociación de variables clínicas con la duración del tratamiento antibiótico. El desenlace primario por evaluar fue la mortalidad a 30 días. Resultados. Se incluyeron 72 pacientes. El 25 % (n=18) recibieron terapia antibiótica por 7 días o menos y el 75 % (n=54) recibieron más de 7 días. No hubo diferencias significativas en la mortalidad a 30 días entre los dos grupos ni en las demás variables estudiadas. La duración de la antibioticoterapia no influyó en la mortalidad a 30 días (OR 0,956; IC95% 0,797 - 1,146). Conclusión. No hay diferencias significativas en los desenlaces clínicos de los pacientes con colecistitis aguda que son sometidos a colecistostomía y que reciben cursos cortos de antibióticos en comparación con cursos largos


Introduction.In patients with acute cholecystitis who receive treatment with cholecystostomy, the optimal duration of antibiotic therapy is unknown. The objective of this study is to compare short courses of antibiotics (7 days or less) with long courses (more than 7 days) in this population. Methods. We performed a retrospective observational cohort study which included patients diagnosed with acute cholecystitis, who received antibiotic therapy and were taken to cholecystostomy. Univariate analysis and logistic regression were performed to evaluate the association between clinical variables and the duration. The main outcome evaluated was 30-day mortality. Results. Seventy-two patients were included, 25% (n=18) were given 7 or fewer days of antibiotics while 75% (n=54) were given them for more than 7 days. Demographic data between both groups were similar (age, severity of cholecystitis, comorbidities). There were no significant differences in 30-day mortality between both groups. Antibiotic duration did not influence mortality at 30 days (OR 0.956, 95% CI 0.797 - 1.146). Conclusion. There are no significant differences in the clinical outcomes of patients with acute cholecystitis who undergo cholecystostomy and receive short courses of antibiotics compared to long courses

ABSTRACT

Bacterial infection is the main cause of infectious diseases in children. Antibacterials play an important role in anti infection treatment of children. At present, the treatment of antimicrobial drugs in children is facing a severe situation of bacterial resistance. In January 2020, a children′s specialized hospital carried out the practice of precise management of antibiotics in combination with key performance indicators. Through the multi sectoral linkage of management and technology, eight key performance indicators and assessment methods were set up from three levels of antibiotic use, bacterial resistance and hospital infection, to standardize the clinical application of antibiotics and continue to promote the rational use of antibiotics. This practice had improved the performance indicators of antibacterial management. Among them, the use intensity of antibacterial drugs for inpatients decreased from 40.07 DDD in 2019 to 29.00 DDD in 2021, the use rate of antibacterial drugs for inpatients decreased from 81.32% to 64.40%, the percentage of antibacterial drug expenses in total drug expenses decreased from 35.41% to 26.82%, the use proportion of non restricted antibacterial drugs in antibacterial drugs increased from 71.30% to 82.21%, and the drug resistance rate of Salmonella to β-Lactam/enzyme inhibitors decreased from 4.84% to 0.03%, and the incidence of hospital infection decreased from 1.16% to 0.96%. The precise management of antibiotics in combination with key performance indicators improved the level of rational use of antibiotics, effectively curbed bacterial resistance, achieved phased results, so as to provide a reference for the scientific management of antibiotics in children′s hospitals.

Introducción: El tratamiento de los canales radiculares de molares temporales con pulpas infectadas ha sido ampliamente descrito y motivo de discusión por muchos años, no existiendo aún un consenso en cuanto al material de obturación. La pasta CTZ (cloranfenicol, tetraciclina y óxido de zinc más eugenol) acompañada de la Técnica de Endodoncia No Instrumentada ha mostrado una alta efectividad clínica y radiográfica para el tratamiento de molares temporales con compromiso pulpar. Se ha propuesto el reemplazo del componente Tetraciclina por Doxiciclina de la formulación, por las implicancias de un posible amelo-génesis imperfecta y la coloración en la corona de dicho componente. Objetivo. Evaluar la efectividad clínica de la pasta CDZ en el tratamiento endodóntico de dientes primarios necrosados con una técnica mínimamente invasiva. Metodología. Estudio de intervención en el que se incluyeron pacientes que presentaban dientes temporales con indicación de terapia pulpar y en quienes se utilizó la pasta CDZ. El éxito del tratamiento se midió por la desaparición de la sintomatología. Resultados. Se incluyeron en el estudio 76 pacientes entre 2 a 9 años. La eficacia del tratamiento con CDZ fue del 97,6% en 125 dientes. Conclusiones. Los hallazgos son comparables a los estudios que utilizaron el CTZ. Debido a las características biológicas del material, su bajo costo, fácil manipulación y excelentes resultados clínicos, se considera una opción en la terapia pulpar en dientes temporales, como una alternativa en el uso de programas de salud pública.


Introduction: Treatment of root canals of primary molars with infected pulps has been widely described and has been the subject of discussion for many years, and there is still no consensus regarding the filling material. The CTZ paste (chloramphenicol, tetracycline and zinc oxide plus eugenol) accompanied by the Non-Instrumented Endodontic Technique has shown high clinical and radiographic effectiveness for the treatment of primary molars with pulp involvement. The replacement of the Tetracycline component by Doxycycline of the formulation has been proposed, due to the implications of a possible amelogenesis imperfecta and the coloration in the crown of said component. Goal. To evaluate the clinical effectiveness of CDZ paste in the endodontic treatment of necrotic primary teeth with a minimally invasive technique. Methodology: Intervention study in which patients with temporary teeth with an indication for pulp therapy and in whom CDZ paste was used were included. Treatment success was measured by the disappearance of symptoms. Results. 76 patients between 2 and 9 years old were included in the study. The efficacy of CDZ treatment was 97.6% in 125 teeth. Conclusions: The findings are comparable to studies using CTZ. Due to the biological characteristics of the material, its low cost, easy handling and excellent clinical results, it is considered an option in pulp therapy in primary teeth, as an alternative in the use of public health programs.

Introducción. La apendicitis aguda es una emergencia quirúrgica frecuente, en la cual el tratamiento de tipo conservador basado en antibióticos se ha identificado como una opción terapéutica que necesita seguir siendo estudiada. El objetivo de este estudio fue determinar las diferencias en seguridad y eficacia del uso de antibióticos en comparación con la apendicectomía en adultos con apendicitis no complicada. Métodos. Revisión sistemática y metaanálisis. Se encontraron 452 estudios; después de una selección, 45 se evaluaron en texto completo y 15 para calidad metodológica; 11 estudios fueron seleccionados y 9 incluidos en el metaanálisis. Resultados. Se contó con 3186 participantes, de los cuales 1512 fueron tratados con terapia antibiótica y 1674 sometidos a apendicectomía. Se identificó estancia hospitalaria más corta en los pacientes sometidos a apendicectomía (SMD: 0,28; IC95%: 0,14 a 0,41). Para los pacientes tratados con terapia antibiótica, se evidenció puntajes de riesgo menores en las escalas de diagnóstico de apendicitis (SMD: -0,13; IC95%: -0,22 a -0,04), menor éxito terapéutico en un 16 % (RR: 0,84; IC95%: 0,77 a 0,92) y reducción del riesgo de complicaciones del 63 % (RR: 0,37; IC95%: 0,25 a 0,53). Se encontró alta heterogeneidad y riesgo de sesgo de publicación. Conclusiones. La terapia antibiótica necesita mayor evidencia para desenlaces como calidad de vida, satisfacción, dolor, o complicaciones específicas, entre otros, que permitan hacer comparaciones más contundentes. Los pacientes que consideren el manejo conservador necesitan ser adecuadamente asesorados y monitorizados para optimizar sus posibilidades de resultados favorables y la oportuna identificación de complicaciones que necesiten de otros abordajes


Introduction. Acute appendicitis is a frequent surgical emergency, in which conservative antibiotic-based treatment has been identified as a therapeutic option that needs further study. The objective of this study is to determine the differences in safety and efficacy of the use of antibiotics compared to appendectomy in uncomplicated appendicitis in adults. Methods. Systematic review and meta-analysis; 452 studies were found; 45 were evaluated in full text, 15 were evaluated in methodological quality; 11 studies were selected and nine meta-analysed. Results. There were 3186 participants, of whom 1512 were treated with antibiotic therapy and 1674 underwent appendectomy. Shorter stays are identified in patients undergoing appendectomy (SMD: 0.28 CI 95% 0.14 to 0.41). It was evidenced for patients treated with antibiotic therapy lower risk scores in the appendicitis diagnostic scales (SMD: -0.13; CI 95%: -0.22 to -0.04), in 16% less therapeutic success (RR: 0.84; CI 95%: 0.77 to 0.92) and a reduction in the risk of complications of 63% (RR: 0.37, CI 95%: 0.25 to 0.53). High heterogeneity and risk of publication bias were reported.Conclusions. Antibiotic therapy needs more evidence for outcomes such as quality of life, satisfaction, pain, specific complications, among others; that allow for more compelling comparisons. Patients who consider the conservative approach need to be adequately advised and monitored to optimize their chances of favorable results and the timely identification of complications that require other approaches

Introducción. Las infecciones del árbol biliar surgen principalmente por estasis asociada a colecistitis aguda, coledocolitiasis y colecistitis alitiásica, siendo pilar del tratamiento un drenaje apropiado y un régimen antibiótico eficaz. El objetivo de esta investigación fue caracterizar la flora bacteriana de los cultivos de bilis. Métodos. Se realizó un estudio observacional, descriptivo, de corte transversal y retrospectivo de pacientes llevados a colecistectomía, colangiopancreatografía retrograda endoscópica y colecistostomía en nuestra institución, a quienes se les tomó cultivo de líquido biliar entre 2017 - 2021. Resultados. Se incluyeron 119 pacientes intervenidos, de los cuales 55,4 % fueron mujeres y el promedio de edad fue de 63,1 (± 16). La clasificación Tokio 2 fue la más frecuente (55,4 %). El germen aislado con mayor frecuencia fue E. coli (51,2 %). El antibiótico más usado fue ampicilina más sulbactam (44,6 %) seguido de piperacilina tazobactam (40,3 %). Conclusión. La Escherichia coli es el germen aislado con mayor frecuencia en infecciones del árbol biliar. No hay claridad con respecto al uso de antimicrobianos de manera profiláctica en esta patología, por esto es conveniente generar protocolos para la toma de muestras y cultivos de bilis en esta población, con el fin de establecer la necesidad del uso de antibióticos y conocer los perfiles de resistencia bacteriana.


Introduction. Infections of the biliary tree arise mainly from stasis associated with acute cholecystitis, choledocholithiasis, and acalculous cholecystitis, with appropriate drainage and an effective antibiotic regimen being the mainstay of treatment. This research is proposed with the aim aim to characterize the bacterial flora of bile cultures.Methods. An observational, descriptive, cross-sectional and retrospective study of patients who underwent cholecystectomy, endoscopic retrograde cholangiopancreatography and cholecystostomy in our institution, whom bile fluid culture was taken between 2017 and 2021, was performed. Results. 119 operated patients were included, of which 55.4% were women, the average age was 63.1 (± 16). The TOKIO 2 classification was the most frequent with 55.4% of the patients. The most frequently isolated germ was E. coli with 51.2%. The most used antibiotic was ampicillin plus sulbactam with 44.6% prescription followed by piperacillin tazobactam with a prescription frequency of 40.3%.Conclusion. Escherichia coli is the most frequently isolated germ in infections of the biliary tree. There is no clarity regarding the use of antimicrobials prophylactically in this pathology. For this reason it is convenient to generate protocols for taking samples and bile cultures in this population, in order to establish the need for the use of antibiotics and to know the profiles of bacterial resistance.

Introducción: Las infecciones recurrentes en el sistema de conductos radiculares son atribuidas principalmente a la presencia de especies como Enterococcus faecalis, lo que hace necesario el estudio de sustancias alternativas al hipoclorito de sodio que puedan ser empleadas como irrigantes de los conductos y mejorar la tasa de éxito de tratamiento. Objetivo: Identificar la composición química del aceite esencial de Origanum vulgare L. (orégano) y evaluar su actividad antibacateriana frente a E. Faecalis ATCC 29212. Métodos: Se diseñó un estudio experimental in vitro. El Origanum vulgare L. se recolectó en la provincia de Tarata, Tacna, Perú. El aceite esencial se obtuvo por método de arrastre de vapor y una muestra fue sometida a cromatografía de gases acoplada a espectrometría de masas en un equipo cromatográfico QP2010 (Ultra Shimazu) equipado con una columna DB-5 MS para identificar sus constituyentes. La actividad antibacteriana del aceite esencial se evaluó a través del método de difusión de disco en agar cerebro-corazón con concentraciones desde 2,261 a 27,132 mg/µL. El cálculo de las repeticiones se realizó con el programa EPi infoTM. Adicionalmente, se identificó la concentración mínima inhibitoria y la concentración mínima bactericida del aceite esencial. Se realizó el análisis descriptivo de los datos y se aplicó la prueba ANOVA de un factor para comparar los valores medios de los halos de inhibición de las distintas concentraciones. Se adoptó un nivel de significancia de p < 0,05. Resultados: Se identificaron 20 constituyentes, siendo los principales compuestos alpha.-pinene (24,44 por ciento) y 1,6-Ocatien-3-ol,3,7,7dimethyl (12,52 por ciento). E. Faecalis fue muy sensible (++) y extremadamente sensible (+++) a concentraciones mayores a 15,827 mg/µL del aceite esencial de Origanum vulgare L. Todas las concentraciones inhibieron el crecimiento bacteriano, mientras que las concentraciones mayores de 14,018 mg/µL fueron bactericidas. Conclusiones: Los principales constituyentes del aceite esencial de Origanum vulgare L. fueron .alpha.-Pinene y Cis-.beta.-Terpineol. Además, se demostró un importante efecto antibacteriano frente a Enterococcus faecalis ATCC 29212(AU)


Introduction: Recurrent infections in the root canal system are mainly attributed to the presence of species such as Enterococcus faecalis. It is therefore necessary to study substances other than sodium hypochlorite which may be used as irrigants for the canals, thus improving the success rate of the treatment. Objective: Identify the chemical composition of essential oil from Origanum vulgare L. (oregano) and evaluate its antibacterial activity against E. faecalis ATCC 29212. Methods: An in vitro experimental study was conducted. Origanum vulgare L. was collected from the province of Tarata, Tacna, Peru. The essential oil was obtained by steam entrainment, and a sample was subjected to gas chromatography / mass spectrometry in a QP2010 chromatograph (Ultra Shimadzu) equipped with a DB-5 MS column to identify its constituents. Antibacterial activity of the essential oil was evaluated by the brain heart agar disk diffusion method at concentrations ranging from 2 261 to 27 132 mg/µl. The software EPi InfoTM was used to estimate the repetitions. Additionally, identification was performed of the minimum inhibitory concentration and the minimum bactericidal concentration of the essential oil. The data were subjected to descriptive analysis and one-factor ANOVA was performed to compare the mean values of inhibition haloes at the different concentrations. A significance level of p < 0.05 was established. Results: Twenty constituents were identified. The main compounds were .alpha.-pinene (24.44 percent) and 1,6-Ocatien-3-ol,3,7,7dimethyl (12.52 percent). E. Faecalis was very sensitive (++) and extremely sensitive (+++) to concentrations above 15 827 mg/µl of the essential oil of Origanum vulgare L. All the concentrations inhibited bacterial growth, and concentrations above 14 018 mg/µl were bactericidal. Conclusions: The main constituents of the essential oil of Origanum vulgare L. were .alpha.-pinene and cis-.beta.-terpineol. Additionally, the essential oil was shown to display considerable antibacterial activity against Enterococcus faecalis ATCC 29212(AU)

@#Abstract: Objective To investigate the distribution and antimicrobial resistance profile of the bacterial strains isolated from blood cultures in neonatal septicemia children of Neonatology Department, the First People's Hospital of Chuzhou during Jan. 2017-Dec. 2021, in order to guide clinical rational drug use. Methods The distribution and the results of antimicrobial susceptibility tests and characteristics of the pathogenic bacteria isolated from blood culture samples in neonatal septicemia children in the First Hospital of Chuzhou from Jan. 2017 to Dec. 2021 were retrospectively analyzed. The results were analyzed with WHONET 5.6 software, according to the Clinical and Laboratory Standards Institute (CLSI) 2021 breakpoints.  Results A total of 189 strains were isolated from the 4 538 sample of blood cultures, the positive rate was 4.2%, including 59(31.2%) Gram-negative bacterial strains, 130 (68.8%) Gram-positive bacterial strains. The most frequently isolates were coagulase-negative staphylococci(64.0%), Serratia liquefaciens (15.9%), Escherichia coli (3.2%), Acinetobacter lwoffii (2.6%) and Delftia acidovorans (2.6%). The prevalence of methicillin-resistant isolates was 81.8%(99/121) in coagulase-negative Staphylococci and 25.0%(1/4) in Staphylococcus aureus. No staphylococcal strains were found resistant to vancomycin, quinupristin-dalfopristin or linezolid. The sensitivity of the antibacterial drug monitored by Serratia liquefaciens was 100.0%.Conclusions Gram-positive bacterial are the main pathogen of neonatal septicemia, and is highly resistant to the common antibacterial drugs. The clinical should choose antibacterial agents reasonably according to drug sensitivity.

ABSTRACT

Introduction: Minthostachys mollis is medicinal plant frequently used because of its anti-inflammatory and antibacterial properties. Objective: Identify the chemical composition of the essential oil from Minthostachys mollis and evaluate its antimicrobial properties against oral pathogens. Methods: The essential oil was extracted from the fresh leaves and branches of the plant by steam distillation method, and Gas Chromatography-Mass Spectrometry were performed to identify the principal constituents of the essential oils. Repetitions for the antimicrobial assay were calculated with the Epi info TM program. The antimicrobial activity was tested against Enterococcus faecalis ATCC 29212, Porphyromonas gingivalis ATCC 33277 and Candida albicans ATCC 10231 by disc diffusion assay and microdilution method, the minimum inhibitory concentration, minimum bacterial concentration and minimum fungal concentration were recorded. Results: The main compounds found were menthone (32.9 percent) and eucalyptol (28.1 percent). The essential oil of M. mollis demonstrated inhibition zones against E. Faecalis, P. gingivalis and C. albicans of 15.13 ± 0.39, 17.96 ± 0.21, and 20.82 ± 0.63 mm, respectively. There were significant differences on the inhibition zones between the three strains tested, being C. albicans the most sensible microorganism (p < 0.05). The minimum inhibitory concentration ranged from 1.05 to 2.11 µg/mL, the minimum bacterial concentration ranged from 2.11 to 4.23 µg/mL, and the minimum fungal concentration was 1.05 µg/mL. Conclusions: The essential oil of Minthostachys mollis showed as principal compounds menthone and eucalyptol and it has an important antimicrobial activity against E. faecalis, P. gingivalis and C. albicans(AU)


Introducción: Minthostachys mollis es una planta medicinal de uso frecuente por sus propiedades antiinflamatorias y antibacterianas. Objetivo: Identificar la composición química del aceite esencial de Minthostachys mollis y evaluar sus propiedades antimicrobianas frente a patógenos orales. Métodos: El aceite esencial se extrajo de las hojas y ramas frescas de la planta mediante el método de destilación al vapor; se realizó una cromatografía de gases-espectrometría de masas para identificar los principales constituyentes del aceite esencial. Las repeticiones del ensayo antimicrobiano se calcularon con el programa Epi infoTM. La actividad antimicrobiana se evaluó frente a Enterococcus faecalis ATCC 29212, Porphyromonas gingivalis ATCC 33277 y Candida albicans ATCC 10231 mediante ensayo de difusión en disco y método de microdilución. Se registró la concentración mínima inhibidora, la concentración mínima bactericida y la concentración mínima fungicida. Resultados: Los principales compuestos encontrados fueron mentona (32,9 por ciento) y eucaliptol (28,1 por ciento). El aceite esencial de M. mollis generó zonas de inhibición frente a E. Faecalis, P. gingivalis y C. albicans de 15,13 ± 0,39, 17,96 ± 0,21 y 20,82 ± 0,63 mm, respectivamente. Existió diferencias significativas en las zonas de inhibición entre las tres cepas, siendo C. albicans el microorganismo más sensible (p < 0,05). La concentración mínima inhibitoria varió de 1,05 a 2,11 µg/mL, la concentración mínima bacteriana varió de 2,11 a 4,23 µg/mL y la concentración mínima de hongos fue 1,05 µg/mL. Conclusiones: El aceite esencial de Minthostachys mollis mostró como compuestos principales mentona y eucaliptol y posee una importante actividad antimicrobiana contra E. faecalis, P. gingivalis y C. albicans.

Introducción: Las propiedades antibacterianas de Plantago major frente a microorganismos orales no se ha estudiado ampliamente. Objetivo: Identificar los metabolitos secundarios presentes en el extracto etanólico de Plantago major y determinar su actividad antibacteriana frente a Streptococcus mutans. Métodos: Se prepararon concentraciones del 25 por ciento, 50 por ciento, 75 por ciento y 100 por ciento de extracto etanólico (EE) de Plantago major. Se cargaron seis discos de papel con 10 µL, 15 µL, 20 µL y 25 µL de cada concentración, haciendo un total de 96 discos. Los discos fueron colocados en placas Petri con agar cerebro-corazón inoculadas con Streptococcus mutans, se empleó clorhexidina al 0,12 por ciento como control positivo. Se calcularon seis repeticiones para cada concentración. Las placas fueron incubadas a 37 °C por 48 horas. La medición de los halos de inhibición se realizó con un compás digital. El análisis estadístico se realizó mediante la prueba ANOVA de un factor seguido por la prueba post hoc de Tukey. Resultados: El análisis químico del extracto etanólico de Plantago major identificó la presencia de terpenos, diterpenos, saponinas, terpenoidales y aceites esenciales. Las concentraciones del 25 por ciento y 50 por ciento no mostraron efecto antibacteriano, los volúmenes mayores de 20 µL de la concentración del 75 por ciento y todas los del 100 por ciento fueron efectivos para inhibir el crecimiento de Streptococcus mutans con halos de inhibición de 8,36 mm a 14,64 mm. La clorhexidina al 0,12 por ciento inhibió el crecimiento de Streptococcus mutans con halos de inhibición de 17,77 mm en promedio, presentando diferencias significativas con todas las concentraciones del extracto etanólico de Plantago major (P < 0,05). Conclusiones: El extracto etanólico de Plantago major presentó derivados de los terpenos y saponinas, y mostró actividad antibacteriana frente a Streptococcus mutans en volúmenes y concentraciones mayores a 20 µL/75 por ciento(AU)


Introduction: The antibacterial properties of Plantago major against oral microorganisms have not been widely studied. Objective: Identify the secondary metabolites present in an ethanolic extract of Plantago major and determine their antibacterial activity against Streptococcus mutans. Methods: The Plantago major ethanolic extract (EE) was prepared at concentrations of 25 percent, 50 percent, 75 percent and 100 percent. Six paper discs were loaded with 10 µl, 15 µl, 20 µl and 25 µl of each concentration, for a total 96 discs, which were then placed on Petri plates with brain heart agar inoculated with Streptococcus mutans. The positive control was 0.12 percent chlorhexidine. Six replicates were estimated for each concentration. The plates were incubated at 37ºC for 48 hours. Inhibition haloes were measured with a digital caliper. Statistical analysis was based on one-factor ANOVA testing followed by Tukey's post hoc test. Results: Chemical analysis of the Plantago major ethanolic extract identified the presence of terpenes, diterpenes, saponins, terpenoids and essential oils. The 25 percent and 50 percent concentrations did not display an antibacterial effect, whereas volumes above 20 µl of the 75 percent concentration and all 100 percent volumes were effective to inhibit Streptococcus mutans growth with inhibition haloes of 8.36 mm to 14.64 mm. 0.12 percent chlorhexidine inhibited Streptococcus mutans with inhibition haloes of 17.77 mm on average, presenting significant differences with all the concentrations of the Plantago major ethanolic extract (p < 0.05). Conclusions: The Plantago major ethanolic extract was found to contain terpene and saponin derivatives, and displayed antibacterial activity against Streptococcus mutans at volumes and concentrations above 20 µl / 75 percent(AU)

ABSTRACT

Resumen La sepsis neonatal constituye una de las principales causas de mortalidad neonatal en los países en desarrollo. Los neonatos, en particular los prematuros, tienen un mayor riesgo de infecciones bacterianas, por lo que el manejo con antibióticos constituye la terapia más frecuente en la Unidad de Cuidado Intensivo Neonatal (UCIN), estimándose en hasta un 70%. La presentación clínica de la sepsis neonatal es inespecífica en muchas ocasiones, lo que hace que la antibioticoterapia empírica se inicie tempranamente para evitar consecuencias deletéreas para los pacientes; su inespecificidad la caracteriza como un reto diagnóstico, por lo que diferentes autores han diseñado estrategias para determinar neonatos candidatos a terapia antimicrobiana. La microbiología permanece en contacto estrecho con la clínica ya que conocer los gérmenes frecuentemente asociados a la sepsis neonatal ayuda a determinar el espectro antibiótico a usar. De igual forma, el conocimiento de la farmacología antibiótica es clave puesto que el tratamiento antibiótico no es inocuo y puede asociarse a aumento de morbilidad o mortalidad. El tiempo de evolución y los factores de riesgo maternos están asociados a los gérmenes responsables esperados, ya descritos por múltiples estudios descriptivos a nivel mundial. El uso indiscriminado de antibióticos de amplio espectro para el manejo de infecciones en los recién nacidos ha generado un aumento en la resistencia antibiótica, lo que a su vez lleva a tasas más altas de fallo terapéutico con el uso de las terapias antibióticas empíricas. A razón de lo anterior, los Programas de Vigilancia de Uso de Antibióticos juegan un papel determinante para monitorear el cambio en la resistencia a nivel local para ajustar y homogeneizar la práctica médica que logre regular el uso de antibióticos y mitigar la emergente resistencia bacteriana.


Abstract Neonatal sepsis constitutes one of the main causes of neonatal mortality in developing countries. Newborns, particularly premature newborns, have a higher risk of bacterial infections that result in frequent administration of antibiotics in the Neonatal Intensive Care Units (NICU), which is estimated to be as high as 70%. The clinical presentation of neonatal sepsis is nonspecific, prompting the early use of empirical antibiotic prescription to avoid adverse consequences in the patients. Its non-specificity characterizes it as a challenging diagnostic, this aspect led several authors to design strategies to determine which newborns are true candidates for antimicrobial therapy. Microbiology is closely linked to clinical practice. Thus, knowing the most frequent bacteria associated with neonatal sepsis will be closely related to the antibiotic spectrum that should be used to treat it. Furthermore, knowledge on basic pharmacology is key inasmuch as the antimicrobial treatment is not innocuous and can be related to an increase in mortality and morbidity. Clinical course and maternal risk factors are associated with the expected responsible germs that are already described in multiple descriptive studies worldwide. Indiscriminate use of broad-spectrum antibiotics for the management of newborn infections is leading to antibiotic resistance increase. At the same time, this is related to even higher rates of therapeutic failure with empiric antimicrobial treatment. Based on this, Antimicrobial Stewardship Programs play a determinant role to monitor the changes in local resistance to adjust and homogenize medical practice to regulate the use of antibiotics and mitigate the emergent and threatening antimicrobial bacterial resistance.

Introducción: Entre las infecciones asociadas con la atención en salud, las relacionadas con dispositivos constituyen la patología más común en los pacientes que ingresan al servicio de cuidados intensivos. Objetivos: Determinar la distribución de las infecciones asociadas con dispositivos, su perfil microbiológico y resistencia bacteriana en las unidades de cuidados intensivos del departamento de Casanare, Colombia entre 2019 y 2020. Materiales y métodos: Estudio observacional retrospectivo de corte transversal. La población de estudio fueron 93 pacientes admitidos entre enero de 2019 y diciembre de 2020 en dos unidades de cuidados intensivos del depar-tamento de Casanare. La información fue suministrada por la Secretaría de Salud Departamental, a través de los reportes de los laboratorios clínicos por medio del programa WHONET 5.6. Resultados: Las infecciones del torrente sanguíneo asociadas con el uso de catéter fueron las más frecuentes, con un 84 % (n: 78); seguidas de las infecciones sintomáticas de las vías urinarias asociadas con catéter, con un 12 % (n: 11), y en menor proporción las neumonías asociadas con ventilador mecánico, en un 4 % (n: 4). Las bacterias gramnegativas presentaron mayor frecuencia (61 %; n: 14) en relación con las grampositivas (39 %; n: 9). Conclusión: Las infecciones más frecuentes en los servicios analizados fueron las del torrente sanguíneo asociadas con el catéter y Pseudomonas aeruginosa fue el microrganismo más prevalente en los tres tipos de infección; mien-tras que el Enterococcus faecium fue resistente a una variedad de antibióticos. Tales resultados, al ser comparados con estudios realizados en varios países, demostraron que la distribución de estas infecciones es variable. Introduction: Among health care associated infections, device associated infections are the most common pathology in patients admitted to the intensive care service. Objectives: To determine the distribution of device-associated infections, their microbiological profile and bacterial resistance, in the intensive care units of the Department of Casanare between 2019 and 2020. Materials and methods: An observational, retrospective, cross-sectional study was carried out. The study population was 93 patients admitted between January 2019 and December 2020 in two Inten-sive Care Units of the Department of Casanare. The information was provided by the Departmental Health Secretariat through the reports of the Clinical Laboratories through the WHONET 5.6 program. Results: Catheter-associated bloodstream infections were the most frequent with 84% (n: 78), fo-llowed by catheter-associated symptomatic urinary tract infections with 12% (n: 11) and associated pneumonia to a lesser extent 4% mechanical ventilator (n: 4). Gram negative bacteria had a higher frequency 61% (n: 14) compared to Gram positive ones 39% (n: 9). Conclusion: The infection associated with devices, the most frequent in the analyzed services were the infections of the blood stream associated with the catheter and P. aeruginosa was the most preva-lent microorganism in the three types of infection. E. faecium presented resistance to a variety of an-tibiotics, results that when compared with studies carried out in several countries worldwide showed that the distribution of these infections is variable.


Introdução: Entre as infecções associadas aos cuidados de saúde, as infecções associadas a dispositi-vos são a patologia mais comum em pacientes internados em terapia intensiva. Objetivo: Determinar a distribuição de infecções associadas a dispositivos, seu perfil microbiológico e resistência bacteriana em unidades de terapia intensiva no departamento de Casanare, Colômbia entre 2019 e 2020. Materiais e métodos: Estudo retrospectivo observacional transversal. A população do estudo foi de 93 pacientes admitidos entre janeiro de 2019 e dezembro de 2020 em duas unidades de terapia in-tensiva no departamento de Casanare. As informações foram fornecidas pela Secretaria de Saúde do Departamento, através de informes de laboratório clínico utilizando o programa WHONET 5.6. Resultado: As infecções da corrente sanguínea associadas a cateteres foram as mais frequentes com 84% (n: 78), seguidas pelas infecções do trato urinário sintomáticas associadas a cateteres com 12% (n: 11), e em menor grau as pneumonias associadas a ventiladores mecânicos com 4% (n: 4). As bac-térias gram-negativas eram mais frequentes (61%; n: 14) do que as gram-positivas (39%; n: 9). Conclusão: As infecções mais frequentes nos serviços analisados foram infecções associadas a cate-teres e Pseudomonas aeruginosa foi o microrganismo mais prevalente nos três tipos de infecção, en-quanto Enterococcus faecium era resistente a uma variedade de antibióticos. Tais resultados, quando comparados com estudos realizados em vários países, demonstraram que a distribuição dessas infe-cções é variável.

ABSTRACT: The present in vitro study determined the antibacterial effect of Coconut (Cocus nucifera) oil on Streptococcus mutans ATCC 25175. Cross-sectional and experimental study. Streptococcus mutans strain ATCC 25175 was seeded on Müeller- Hinton Blood Agar. The effect was determined by inhibition halo and the minimum inhibitory concentration, using procaine penicillin G as a positive control and standard suspension of Streptococcus mutans as a negative control. Twelve replicates per Coconut oil concentration (25 %, 50 % and 75 %) were performed. Coconut oil concentration at 25 % generated an inhibitory mean of 17 mm and 2.23x102 CFU, at 50 % a mean of 21.75 mm and 0.17 x 102 CFU, at 75 % a mean of 22 mm and 0 CFU, procaine penicillin G a mean of 14.25 mm and 0 CFU, the negative control gave a mean of 2.8 x 10 5 CFU. The Kruskal-Wallis non-parametric test detected a highly significant statistical difference of the three concentrations of Coconut oil (p < 0.01). The Mann-Whitney test with Bonferroni adjustment determined that the 50 % and 75 % concentration had similar inhibitory action and that both the 75 % concentration and procaine penicillin G gave a mean of 0 CFU. All concentrations of Coconut oil showed inhib itory action on Streptococcus mutans ATCC 25175. 75 % concentration showed the highest inhibitory mean and was the minimum inhibitory concentration that completely eliminated CFU.


RESUMEN: El presente estudio in vitro determinó el efecto antibacteriano del aceite de coco sobre Streptococcus mutans ATCC 25175. Estudio transversal y experimental. La cepa de Streptococcus mutans ATCC 25175 se sembró en Agar Sangre Müeller-Hinton. El efecto se determinó mediante el halo de inhibición y la concentración inhibitoria, utilizando la penicilina G procaína como control positivo y la suspensión estándar de Streptococcus mutans como control negativo. Se realizaron doce réplicas por cada concentración de aceite de coco (25 %, 50 % y 75 %). La concentración de aceite de coco al 25 % generó una media inhibitoria de 17 mm y 2,23 x 102 UFC, al 50 % una media de 21,75 mm y 0,17 x 102 UFC, al 75 % una media de 22 mm y 0 UFC, la penicilina G procaína una media de 14,25 mm y 0 UFC, el control negativo dio una media de 2,8 x 10 5 UFC. La prueba no paramétrica de Kruskal-Wallis detectó una diferencia estadística altamente significativa de las tres concentraciones de aceite de coco (p < 0,01). La prueba de Mann-Whitney con ajuste de Bonferroni determinó que las concentraciones del 50 % y del 75 % tenían una acción inhibidora similar y que tanto la concentración del 75 % como la penicilina G procaína daban una media de 0 UFC. Todas las concentraciones de aceite de coco mostraron una acción inhibitoria sobre Streptococcus mutans ATCC 25175. La concentración del 75 % mostró la media inhibitoria más alta y fue la concentración inhibitoria mínima que eliminó completamente las UFC.

Introducción: El aceite esencial de Minthostachys mollis ha demostrado poseer importantes propiedades antimicrobianas. Objetivo: Caracterizar químicamente las fracciones obtenidas del aceite esencial de Minthostachys mollis y evaluar la actividad antimicrobiana sobre Streptococcus mutans, Lactobacillus acidophilus, Enterococcus faecalis, Porphyromonas gingivalis y Candida albicans. Material y Métodos: Las fracciones de éter de petróleo, diclorometano y metanol del AE de M. mollis fueron caracterizadas químicamente por cromatografía de gases acoplada a espectrometría de masas. Las repeticiones del ensayo antimicrobiano se calcularon con el programa EPIDAT v.4.2. La actividad antimicrobiana se realizó por el método de difusión de disco y se calculó la concentración mínima inhibitoria por el método de microdilución. Los datos fueron analizados empleando la prueba ANOVA (p=0,05). Resultados: Los principales constituyentes de las fracciones de éter de petróleo, diclorometano y metanol fueron cis-Menthone (39,8 por ciento, thymol (31,2 por ciento) y α-Terpineol (43,6 por ciento), respectivamente. Todas las cepas fueron sensibles a las tres fracciones, aunque C. albicans fue la cepa más sensible, registrando halos de inhibición de 14,73±0,57 mm para la fracción de metanol, 20,91±0,55 mm para éter de petróleo y 20,38±0,58 mm para diclorometano, se encontraron diferencias significativas cuando se compararon frente a Clorhexidina al 0,12 por ciento y Nistatina (p<0,05). Las concentraciones mínimas inhibitorias de las fracciones variaron de 0,2 a 3,2 µg/mL. Conclusiones: Los principales constituyentes de las fracciones de éter de petróleo, diclorometano y metanol fueron cis-Menthone, thymol y α-Terpineol. Las fracciones de éter de petróleo y diclorometano fueron altamente efectivas para inhibir el crecimiento de S. mutans, L. acidophilus, E. faecalis, P. gingivalis y C. albicans(AU)

ABSTRACT

Introducción: Las plantas medicinales han demostrado poseer propiedades antibacterianas para el control de la periodontitis. Objetivo: Determinar la actividad antibacteriana frente a Porphyromonas gingivalis ATCC 33277 de un gel experimental compuesto por aceite esencial de Eucalyptus globulus Labill. Métodos: Se realizó un estudio experimental in vitro. Se empleó el programa EPi InfoTM para el cálculo de las repeticiones. El aceite esencial se obtuvo por el método de arrastre de vapor; se identificó su composición química por cromatografía de gases acoplada a espectrometría de masas. Se evaluó la concentración mínima inhibitoria (CMI) y concentración mínima bactericida (CMB). Se realizó un ensayo de difusión en Agar para medir los halos de inhibición del gel experimental al 4,46 por ciento frente a P. gingivalis, la comparación con clorhexidina al 0,12 por ciento se evaluó con la prueba U de Mann-Whitney. Se adoptó un nivel de significancia del 5 por ciento . Resultados: Se identificaron 11 constituyentes en el aceite esencial, los principales componentes químicos fueron 3-heptadecene, (Z)- (36,13 por ciento ), 1-tridecene (14,7 por ciento ) y 1,8-cineole (9,72 por ciento ). La CMI del aceite esencial fue 36,195 mg/mL y la CMB fue 39,114 mg/mL. Los halos de inhibición del gel experimental de P. gingivalis fueron 25,533 mm ± 0,960. mm. Se observaron diferencias estadísticamente significativas frente a clorhexidina al 0,12 por ciento (23,282 ± 0,345) (p < 0,05). Conclusiones: El gel experimental al 4,46 por ciento compuesto por aceite esencial de Eucalyptus globulus Labill presentó una actividad antibacteriana importante frente a Porphyromonas gingivalis ATCC 33277(AU)


Introduction: Medicinal plants have proved to have antibacterial properties for the control of periodontitis. Objective: Determine the antibacterial activity against Porphyromonas gingivalis ATCC 33277 of an experimental gel composed of essential Eucalyptus globulus Labill oil. Methods: An in vitro experimental study was conducted. The software EPi InfoTM was used to estimate the repetitions. The essential oil was obtained by steam entrainment, and its chemical composition was determined by gas chromatography / mass spectrometry. Minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) were also evaluated. An agar diffusion test was performed to measure the inhibition haloes of the 4.46 percent experimental gel against P. gingivalis. Comparison with 0.12 percent chlorhexidine was evaluated with the Mann-Whitney U test. A 5 percent significance level was adopted. Results: A total 11 constituents were identified in the essential oil. The main chemical components were 3-Heptadecene, (Z)- (36.13 percent), 1-Tridecene (14.7 percentand 1,8-cineole (9.72 percent). MIC of the essential oil was 36.195 mg/ml, whereas MBC was 39.114 mg/ml. The inhibition haloes of the experimental P. gingivalis gel were 25.533 mm ± 0.960 mm. Statistically significant differences were observed versus 0.12 percent chlorhexidine (23.282 ± 0.345) (p < 0.05). Conclusions: The 4.46 percent experimental gel composed of Eucalyptus globulus Labill essential oil displayed considerable antibacterial activity against Porphyromonas gingivalis ATCC 33277(AU)

Las reacciones adversas a medicamentos son una de las principales causas de muerte en el mundo, producen muchos ingresos hospitalarios y aumentan los costos de atención. Dentro de los medicamentos que más se asocian con estas reacciones están los antibióticos y de estos los más comunes son los betalactámicos, ampliamente utilizados en las instituciones de salud. Las manifestaciones más frecuentes de las reacciones adversas a betalactámicos son alérgicas, dermatológicas, gastrointestinales, renales, hepáticas y neurológicas. Se realiza una revisión general de las reacciones adversas de estos medicamentos, se mencionan los distintos antibióticos betalactámicos con su clasificación y espectro de acción y más precisamente se explican las distintas reacciones adversas por uso de betalactámicos según el sistema comprometido.


Adverse drug reactions are one of the leading causes of death in the world. They are also responsible for an increase in hospital admissions and higher care costs. Among the most associated drugs with these reactions are antibiotics and of these the most common are beta-lactams, which are widely used in health institutions. The most fre-quent manifestations of adverse reactions to beta-lactams are allergic, dermatological, gastrointestinal, renal, hepatic and neurological reactions. A general review of the adverse reactions to these drugs is carried out. Also, the different beta-lactam antibiotics are described along with their classification and spectrum of action, and an accurate explanation of the different adverse reactions due to the use of beta-lactams according to the compromised system is made.


As reações adversas a medicamentos são uma das principais causas de morte no mundo, resultam em muitas admissões hospitalares e aumentam os custos do atendimento. Entre os medicamentos que mais se associam a essas reações estão os antibióticos e, destes, os mais comuns são os beta-lactâmicos, amplamente utilizados em instituições de saúde. As manifestações mais frequentes de reações adversas aos beta-lactâmicos são alérgicas, dermatológicas, gastrointestinais, renais, hepáticas e neurológicas. Faz-se uma revisão geral das reações adversas desses medicamentos, são mencionados os diferentes antibióticos beta-lactâmicos com sua classificação e espectro de ação, e mais precisamente explicam as diferentes reações adversas devidas ao uso de beta-lactâmicos de acordo com o sistema comprometido

RESUMEN Introducción: la infección del tracto urinario es entre las enfermedades infecciosas una de gran incidencia en el adulto y la segunda con mayor incidencia en niños. Su tratamiento es inicialmente empírico y se sustenta fundamentalmente en la epidemiología y susceptibilidad local. Objetivos: identificar los microorganismos causales, la resistencia a antimicrobianos y el tratamiento prescrito empíricamente a pacientes consultantes por infecciones del tracto urinario con bacteriuria asociada en la unidad de emergencia del Hospital Comunitario de Bulnes, Chile, entre enero y junio del 2019. Metodología: se realizó un estudio descriptivo retrospectivo. Los datos fueron obtenidos desde los registros del Hospital comunitario de Bulnes, Chile, utilizando el código CIE-10: N39.0, de donde se obtuvo el resultado de los exámenes, datos clínicos, y el perfil biodemográfico de la muestra utilizando un instrumento construido por los autores. Resultados: el microorganismo aislado con mayor frecuencia fue Escherichia coli, en esta bacteria el mayor porcentaje de resistencia se asocia a ampicilina y ciprofloxacino. Los antibióticos utilizados para el tratamiento empírico de estas infecciones fueron principalmente cefadroxilo y ciprofloxacino. El inicio del tratamiento en su mayoría fue sin resultado de urocultivo y sin cambios posteriores al resultado. Conclusiones: en el hospital comunitario de la región de Ñuble, la infección urinaria presentó una alta incidencia en mujeres, con un cuadro clínico caracterizado por disuria, no siendo excluyente para otra sintomatología. La bacteria preponderante fue E. coli, que mostró un alto porcentaje de resistencia a 1 o más antimicrobianos, siendo la ampicilina y el ciprofloxacino los antibióticos con mayor frecuencia en este sentido. No se logró evidenciar errores en la continuidad en el manejo clínico.


ABSTRACT Introduction: Urinary tract infection is one of the infectious diseases with a high incidence in adults and the second with the highest incidence in children. Its treatment is initially empirical and is fundamentally based on epidemiology and local susceptibility. Objectives: To identify the causative microorganisms, antimicrobial resistance and the empirically prescribed treatment to patients consulting for urinary tract infections with associated bacteriuria in the emergency unit of the Community Hospital of Bulnes, Chile, between January and June 2019. Methodology: A retrospective descriptive study was carried out. The data were obtained from the records of the Community Hospital of Bulnes, Chile, using the ICD-10 code: N39.0, from which the results of the examinations, clinical data, and the biodemographic profile of the sample were obtained using an instrument constructed by the authors. Results: The most frequently isolated microorganism was Escherichia coli, in this bacterium the highest percentage of resistance is associated with ampicillin and ciprofloxacin. The antibiotics used for the empirical treatment of these infections were mainly cefadroxil and ciprofloxacin. The start of treatment was mostly without urine culture results and without changes after the result. Conclusions: In the Community Hospital of the Ñuble region, urinary infection had a high incidence in women, with a clinical picture characterized by dysuria, not excluding other symptoms. The preponderant bacterium was E. coli, which showed a high percentage of resistance to one or more antimicrobials, and ampicillin and ciprofloxacin were the most frequent antibiotics in this regard. It was not possible to show errors in continuity in the clinical management.

Background: In the present study, we have tested whether specimens of the medically relevant scorpion Tityus pachyurus, collected from two climatically and ecologically different regions, differ in the biological activities of the venom. Methods: Scorpions were collected in Tolima and Huila, Colombia. Chemical profiles of the crude venom were obtained from 80 scorpions for each region, using SDS-PAGE and RP-HPLC. Assays for phospholipase A2, direct and indirect hemolytic, proteolytic, neuromuscular, antibacterial, and insecticidal activities were carried out. Results: The electrophoretic profiles of venom from the two regions showed similar bands of 6-14 kDa, 36-45 kDa, 65 kDa and 97 kDa. However, bands between 36 kDa and 65 kDa were observed with more intensity in venoms from Tolima, and a 95 kDa band occurred only in venoms from Huila. The chromatographic profile of the venoms showed differences in the intensity of some peaks, which could be associated with changes in the abundance of some components between both populations. Phospholipase A2 and hemolytic activities were not observable, whereas both venoms showed proteolytic activity towards casein. Insecticidal activity of the venoms from both regions showed significant variation in potency, the bactericidal activity was variable and low for both venoms. Moreover, no differences were observed in the neuromuscular activity assay. Conclusion: Our results reveal some variation in the activity of the venom between both populations, which could be explained by the ecological adaptations like differences in feeding, altitude and/or diverse predator exposure. However more in-depth studies are necessary to determine the drivers behind the differences in venom composition and activities.(AU)
